Work Visa Application
Saudi Embassy, Washington DC 

Passport

(NOT A COPY)
If you do NOT have passport, contact us ASAP.  WE will refer to you to our agent who can expedite the process.
Make sure that your passport is valid for at least 6 months & that there are at least 4 blank pages.

Medical Report

  • Make 3 copies of the form to take to your doctor.
  • Make an appointment ASAP with your family doctor to complete the medical
  • Please note the lab work that will be required. There is a lot.
  • Three copies of the lab results (2 to send along with your application and 1 to take with you).
  • The Medical Report must be completed by your family physician.
  • Make sure that your physician provides her state license number number on
    the medical form under her signature.
  • Immunizations: Currently the only immunizations that is suggested for most hospital workers is for Hepatitis B. You can contact the US Center for Disease Control but most people do not get any immunizations. Consult with your physician. Perhaps the H1N1 would be a good idea this year.

Police Clearance Report

Go to your local police department and request a clearance report. If they have no record, request a signed letter on police department letterhead indicating that they have no record of any offenses. If there are offenses ask for a report and call me immediately.
  • Only a local police report are required. (finger prints are not necessary per the embassy but the police station may require them).
  • The embassy is no longer accepting police clearance reports from the
    internet.

Visa Application

  • Form can be downloaded from the web site
  • Please complete the top of the visa application form and be sure to SIGN IT.
  • Any item that you are unsure of leave blank.
  • There are ONLY 2 answers to the religious question....Christian or Muslim. If you do not wish to indicate one or the other please let us know. We will not be able to get a visa. If you leave it blank we will pick one or the other for you.

    Profession: Teacher

    Qualification: Bachelor Degree

    Departure: Your destination is Jeddah or Riyadh Saudi Arabia, Please indicate a departure date 10 days from the date of the application.


  • Please carefully read and sign the statement of understanding at the bottom of the form. (if it is there).

Photographs:

Send 4 passport size photographs with a white background, full front face view, to Our Agent with your visa application package. (1 for the visa application and 3 for the medical forms)

DO NOT SEND ANY DOCUMENTS DIRECTLY TO THE EMBASSY!

We will provide the name and address of our agent at a later date.
